The heart of the role
As a Care Manager, you’ll lead with purpose - supporting your team and making a meaningful difference to the lives of our residents every day. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance.
Duties include
- Oversee the daily clinical operations of the care home ensuring our residents receive the best care available.
- Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.
- Review clinical incidents, complaints and changes, providing solutions to reduce any risks.
- Lead compliance with AN-ACC assessments.
- Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.
- Fostering a strong team environment.
- Prepare and conduct clinical governance audits.
- Provide insights and innovative solutions for continuous clinical improvement.
- Oversee medication management in accordance with resident needs and legislative requirements.
- Liaise with allied health, medical practitioners and other services to provide optimal resident care.
What will you bring?
- Bachelor of Nursing with current APHRA is essential.
- Post grad qualification in aged care highly regarded.
- Strong and effective leadership, communication and interpersonal skills.
- A person-centred care mindset.
- A passion for nurturing a strong team environment.
- Ingrained passion and empathy for our residents.
- Proven track record in delivering quality clinical care within an aged care or acute setting.
- Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
- Proven and demonstrated experience in fostering a reporting culture balanced with your effective investigative skills.
- An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.
